Clinic participants must bring the following:

  • Permanent Resident Card (Green Card)
  • Drivers License
  • Social Security Card
  • 2 Color Passport type photos
  • All your passports
  • Most recent tax return
  • List of home addresses (5 Years).
  • List of employers (5 years)
  • Dates you've been outside the U,S.
  • List of countries you visited
